Is this a fresh install or a system that has gone through upgrades?

I have seen this rarely in the past but only through upgrades and possibly down to changes Sage did over time and the new files not existing, possibly failed to be copied in during the installing of the new version. 

In theory you could just install CRM somewhere and grab the Icons folder from the fresh install and copy it over (if you're a business partner) if a customer, please ask your business partner to send you a copy of that folder. 

Hi Matthew - sorry for the late reply.  I tried to update this post yesterday, and again just now, but I keep getting an error.  In any case, yes, the DB has gone through numerous upgrades and customizations.  So likely one of those screwed things up.  I was able to resolve pretty much as you suggested, by loading up a generic install. 

Here's the SQL SELECT I used against the generic install 

select CT.tabs_caption, CT.tabs_action, CT.Tabs_Bitmap, * from custom_tabs CT
     where CT.tabs_entity = 'find' and CT.tabs_action like '%search'
     order by CT.tabs_caption

If anyone's interested, here's a link to a screenshot the UPDATES I used against the problem install.  No matter how I tried to post these, e.g. Insert --> Code, Insert --> Picture, etc. - the forum gave an error. Oh well.
