Start with a Right-click on the icon and select Restore Defaults.

Intermittent Errors like that difficult to determine the root cause.  However I would first suggest upgrading to a supported version of Sage 300.. 2021 PU4 is 4 releases back from current 2024 release, but PU4 is 4 PU's behind the most recent version (PU8)..
