I am trying to activate the sql version of OPS Inquiry.
However, it comes up with 
"Connection to Failed. Please make sure you have the correct username and password."

All other modules activated fine. 

We had successfully activate the OPS Inquiry in another database set with the same sql

Please help thanks.
