UIF and Maternity leave dates

I had a staff member go on maternity leave.

1) VIP's UIF reports have the start and end dates reversed on their report.

2)She is no longer on maternity leave, and no maternity leave was recorded for this month, but the UIF reports still has a start and end dates for the maternity leave.

3) Should the start date not be the start of her employment with us.

None of this is making sense to me, I would really like some help here, as August was a complete normal working month for her from beginning to end on full pay. The UIF report is showing her earnings and UIF amount - it's just the dates that are weird and UIF will send me a reply again that there is an error.
