Timesheet Import using CSV template

Does anyone have any instructions on how to use this Timesheet import feature?  I have been playing with the template but I can not figure out how it works.  The instructions show the basic formatting but how do you enter regular and over time hours if it will only allow one type of hour column?  There are no instructions on how the actual data should be entered into the CSV file for all the possible pay types like over time, stat or vacation pay.  I did get it to the point where it would allow me to map columns but I couldn't add more than regular hours.
