I just did the same test as you in Sage 50 2020 and I was able to get Field 1 to show on the form.  It is possible you are testing by emailing but didn't change the email version of the file to the new format and only changed the printed version (or the other way around)?  See the last column in the pdf attached.

No, sorry, that is not part of the internal forms designer.  In order to get this kind of capability, you have to go to external reporting.  I've done this quite a bit with Crystal Reports over the years but to view and print the forms you have to use an external program.  They will not print directly from Sage 50.  It's not hard to do but it is an additional cost and complexity.

It is also possible to program Excel's VBA to pull the data and format any way you want.  However, if you don't have experience with this kind of thing, then you would have to hire someone to do it for you.
