I am trying to print T4 from Sage Reports.
On form , its not printing Full address and Employers account number.
Please advise where I am missing some thing

The full address and account number is only required to be on the copy going to the CRA. If filing on-line you will get this same information on the company copy. It is not required and therefore not printed on the double copy going to the employee.
You are not missing something on this issue.

Is there any way that I can print T4 on CRA provided template from Sage, currently I am using copies to print . Like a built in template in Sage that can give me printed T4 .

You can print T4 forms from Sage 50 either on Plain Paper or on Preprinted Forms. The setting can be found under the Set Up Tab and then Reports and Forms, go to Government Forms and Federal Payroll.
I prefer to use the plain paper option as the preprinted forms do not fit the numbers fully into the boxes. With plain paper all of the boxes and lines are filled in properly.
