We have some staff on Supplementary unemployment benefit plan,

The Service Canada Website says we must provide a T4A slip to include the amount of SUBP that was paid to the emploee, however I can not seem to make T4A for employee's only for vendors. Any assistance on this?

Declare under Code 152 – SUBP qualified under the Income Tax Act in the "Other information" section of the recipient's T4A slip any top-up amounts paid under a SUBP that qualifies as a SUBP under the Income Tax Act.
