Which CRM version you are using? This functionality is available in version 7.1 SP2 and 7.2. Select New Task option instead of Appointment directly and select action as “Meeting”. At right hand side you will notice “Add Attachment” option through which you can upload any document against that particular appointment.

Second way is while uploading document against any standard entity like Company, Person , Opportunity etc, you will notice check box named as “Create Communication and save merge document”. You need to check this checkbox and select action as “Meeting”.
