CRM 7.2b

When I create an appointment and need to add a person, there's a nice little "New Person" button so I click it and a pop-up screen asks for a name, so I type one in and click save. I get a red bar that says "Validation Errors - Please correct the highlighted entries" yet nothing is highlighted and the required entries (the ones with *) are filled.

On some PCs (not all) to the right of the address in the URL bar there's a little page that's ripped in half and when I click it the page reloads, erases the information, and highlights the required fields.

How can I fix this?
