Is it possible to customize the CTI Call Screen?
Not in any great way. You should be able to add Client Side code into the page to change the behaviour but for sophisticated behaviour you will need to look at the QGate, Enbu or other partner addons.
Thanks Jeff, We have had Qgate and just ripped out Alphalogix so don't want to use a 3rd party. Shame the standard screen isn't better, like always fixed to the top of the screen, or slides out from somewhere.
Where would the client side code go?

If you are using Sage CRM 7.2 onwards, you can add a file in to the custom js folder that is automatically included as a JavaScript library within all the Screens. This file can then look at the action code (ACT) to know when it should execute and make the changes you want.

You might want to consider a 3rd party CTI connector for that. You can check out Tenfold. Some of its main features are click to dial, floating UI and call analytics to name a few. It's easy to set up too. You can check them out on the link below.
