Advanced Customization Entity Wizard: issues with Recent List and images

I've recently added several new entities to a demo CRM, all using the Advanced Customization Entity Wizard.  It seems that half the time everything works fine, but it's not uncommon that the new entities do not display in the Recent List after navigating to the Summary page.  Similarly, although I've replaced all of the Summary and small_ gifs, sometimes the icons display and sometimes they don't.

I'm following identical procedures for these new entities, so it seems there's no rhyme-or-reason as to why they don't work correctly 100% of the time.

Just wondering if anyone has seen similar behavior with the results of the adv entity wizard.  I've had this intermittent problem since 2017, with the latest in 2019 R2.

    Graham...also, I took a quick look at this while I was working on this client and it looks like I could not find the correct translation.  Therefore, instead of doing anything in translations, I am having the invo_reference field "auto" populate into the invo_name field so the "name" field is not empty.  Once the "name" field was not empty, the record showed up in the recent list.

    If anyone does know how to change the field from the "name" field to something else so that the record shows up in the recent list, I would love to know how to do it as I ran out of time to figure it out.
