Where are custom office customizations stored?

We have lost our customized SO order entry screen. How we can recover the customized screen?

In \MAS90\SO\Custom\###-### I see so_salesorder.bak and so_salesorder.m4l
The bak file is dated 11/11/2014 and the m4l file is dated 7/22/2015.

Restored the files and life is good again.

Just to clarify, you renamed the .bak to .m4l to fix this?

No, restored the m4l file from a backup. The bak file does not have the latest changes.
