Ops Inquiry Procedure

Im trying to activate Ops Inquiry in Sample company. But when doing that, only in that company header desktop images dissapear from UI and if I tried to run any Icon I get Cannot run UI !!! Do I need to do something else? Create a UNI database for that company? Not sure whats the procedure. A customer send me a DBdump to install here and also use Ops Inquiry and I get same errors so something its missing for sure.

    I dont have any in Sample. So I check my customer database and I found those records. So I delete and I can open company without any issue and icons without any error....BUT once I activate Ops Inquiry there I get those problems.....image header desktop menus dissappear and can't run any icons....strange....I will reinstall Ops Inquiry to check if that helps......
