Create your form and save it with your unique name! 

If you want to create any custom form or template, all you have to do is choose to design any form that looks close to the way you want it to look, and then save that form as a New Named form. After you save it, still in the design mode at the top of the form you'll see the new named form.  Make your changes to that form and save again.
