How to create a simple deduction in formula (hours x $2.00)

Hi, we have 1 employee that we would like to simply deduct $2.00 for each hour s(he) works. This is her way to reimburse us for a small portion of her benefits. We need this to print on her paystub and I assume we will print this to box 12 or box 14 of her W2 at the end of the year. I am familiar with formulas however not sure how to create this formula. Can anyone help me on this??? It would considered a deduction, not a tax.
