Another possibility is to have your Transmittal form customized to allow you to select the address to use.  Because these are Crystal reports, there is the ability to suppress or print certain areas based on some field in the data.  One way would be to create custom field on the Transmittal (for example, FedEX?) then have the report choose which address to print depending on the value in the custom field.
