Printing a statement showing a summary of billings

I would like to print a statement for one client showing the original billing amount of $375.00 in April, plus a line-by-line summary of the monthly interest charges that have accumulated each month since June.  And at the bottom of the statement, it would show the current total balance due.  This statement would be mailed to the client.

(Note, we DO have our bill layout set to show the Aged Balances table at the bottom of the statement, so I am aware that this table IS showing the monthly interest amounts at 30/60/90/120 days.  However, I am looking to show this data in the body of the statement instead, in a line-by-line summary format --- the date and corresponding interest amount.)

How would I do this?  Thank you.
