New Absolute Flat Fee - don't see "Start Another Flat Fee" button

I am running Timeslips 2015 and am having a problem with the Absolute Flat Fee. I need to change the flat fee amount. When I try to do that, it tells me I have already printed a bill using this arrangement. First, this is strange because I have not - I even checked my AR list to make sure. I did switch back and forth between trying flat fee and slips, but never approved any bills. Anyway, so then when I try to change the amount I get the message saying I have already printed a bill using this arrangement and I can click Amount Calculator or Start Another Flat Fee. I don't see either of those buttons. I know I've used Start Another Flat Fee several times in the past, and am pretty sure it's just in the same window titled "Absolute Flat Fees - Fees" with the Amount, Duration and Bill Arrangement Message. Where is the darn button? Does anyone have a screenshot for me, and more importantly - a reason it might have disappeared?

I'm wondering if this is related - I'm pretty sure there used to be a tool bar in the Client List window that isn't there anymore! It included the plus symbol for add a new client. Right now I have no tool bar, just the tabs. What happened?!

    Two totally different issues. To fix the toolbar go to preferences - under interface click on appearance and click on Restore all dialog settings to their defaults.
    For the flat fee, change the status of the flat fee to complete. Save and then you should be able to start a new flat fee.

    Thank you. Yes, I was able to change the Preferences. But for the Flat Fee I don't see anywhere I can change status. I have done that before but don't see it now. Also, under Details it says "Completed: Charges are from slips alone" even though I don't have an invoice in the AR list that shows a bill. I don't see any "Status" button or link, etc. where I can change that.
