The institution number is part of the banking account number, branch # etc.  Is there a new employee that has been added that the information was given incorrectly from the employee?

Get your employee to go to their bank and request the form required for direct deposit information (or a void cheque.) I never work from hand written banking information. Also, HSBC was recently acquired by another bank and maybe their institution numbers have already been changed.
