PO printing uses the line cost.  The line cost is determined by the hierarchy.

The next question is if we are purchasing product from a vendor you wouldn't used the average price to pay for their product you pay what they are selling it for so why would the PO printing even look at that cost. Is seems like this cost is more for cost of inventory not buying product. 

So what can I do for the PO printing not to look at Avg Cost or is that too simple.. 

Standard cost is how you value your inventory.  PO cost is what you pay to the vendor, which can change from transaction to transaction.  Check the vendor last cost.  That is the usual spot where PO costs come from, which makes sense (to default to the unit cost you last purchased that product from that vendor).

Don't get stuck on Average Cost.  PO printing gets the unit cost from the PO line.  Average cost has absolutely nothing to do with PO printing at all (unless you have customized the PO printing form).

I attached the screenshot of PO form with unit cost and screenshot of item in item maintenance that is in question. Also the bottom item is using last cost but doesn't have an average cost at all in item maintenance..  It just seems odd..

I am using a custom form but didn't know actually where to look to see if there was a formula. I did go to the unit cost field in Designer and click find in formula but it was the same formula as what the Standard form had.

Stop looking at Item Maintenance.  Look at the PO line costs.

Ok after a couple of days I see what your saying I found the Vendor last cost and was what appeared on PO form. We actually had another of the same scenario today where we had a lesser amount on a PO because of the last cost was lower then current cost. 

So, is there a way to notify user who enters item on PO that the cost is not correct so they can adjust before printing and send to vendor. 

You should be able to add the Standard Cost field to display in the PO line grid, for easy comparison.  (This won't be overly useful if you use UoM conversion though).

If you want a pop-up warning, that would be a script.
