The form contains custom code and references a component from BHE Consulting, so you should consider contacting them or your reseller about the problem. This type of error generally indicates that either a component was not installed, is corrupted or just unreachable. If it is occurring on a particular machine, reboot and retry if you didn't perform a recent installation. You might also try reinstalling the custom components, especially if this is a recent installation on this machine.
