Unfortunately, I doubt someone would "share" a certified report with you due to the fact that their companies pay thousands of dollars to have consultants design these reports for them. Additionally, programming would need to happen at the client level to add and map custom fields in addition to their specific Fringe ID's to properly fill the form in.  I suggest contacting your consultant to quote the project for your company based on your dataset.
