FUP Report not working

Dear Sagians,

I am setting up reminder campaign for sending mail to all customers. Everything is set but the mails are not triggering. When i run the FUP report manually the below error is getting.

Please let me know if any solutions for this.

Based on your screen shot it looks like you're missing some required values to generate the report. Choose a Customer and Site in your empty "First value" fields in the parameter table.
