Application error when user do manufacturing

My user is facing a challenge when he does manufacturing for one site only. It happens mostly when he enters multiple lines (more than 5). If he does the same for other sites or he does less than 5 lines they go through and also its not consistent sometimes they go through. Appreciate if someone can assist me to resolve this.

What is the context? Work order creation with addition of unplanned material?

its a normal work order creation. Product to be produced is selected and BOM auto loaded, when clicks OK error pops-up.

Did you check if the record already exists in the table MFGMAT?
Can you check the sequence counter assigned?

Else, it seems a bug you should report to support.

I don't have the skills to troubleshoot it.

yes I checked sequence and even tried to assign the next figure manually but couldnt fly. I will check with support, my main concern was why it happens only at one site only hence felt it might have to do with some setup at particular site.
