I find that sometimes dates cause issues here.  So if you changed the date to a future date in your bank rec, and then reverted - if you 'cleared' something while you were in the other date, and if you didn't update the cleared date before reverting, it can cause this to happen.

Also, sometimes, when posting date and transaction date aren't the same, this can also cause this issue in my experience.
