This is typical when you dump / load from a database having Ops inquiry activated.

The quick and dirty solution is as follows (warning - backup your database before making any changes)

1. Open SQL Management Studio and expand your company database / tables 

2. Right-click on the CSAPP table and select Edit Top 200 Rows

3. Locate the row where SELECTOR = UQ

4. Delete the row

5. Log back into the company and activate Ops Inquiry

Question though: All three companies share the same XXXSYS database, If UQ needs to be deleted and reactivated, how come other two companies are working fine?

The activation of modules is recorded within the company and not the system database.

since activation is not in CSAPP system database. shouldn't we fix it in SPECIFIC company data table, somewhere?

Yes - you need to expand the company database you're having the issue with and locate / edit the CSAPP table
